Supported by the HSBC Hong Kong Community Partnership Programme, this project aims to promote the development of a compassionate community in Kwun Tong. We are dedicated to providing comprehensive support to elders aged 55 or above in the district and their caregivers through three main dimensions: "Community Collaboration", "Palliative Planning", and "Palliative Care".
Our services include Advance Care Planning (ACP) discussions, life-and-death education seminars, palliative cafes, and intergenerational art workshops designed to reduce public taboos surrounding death. Additionally, we provide casework services for terminally ill elders in need and train a "Life Companion" palliative volunteer team to conduct home visits. By connecting community stakeholders to build a mutual support network, we ensure elders can walk through a "Journey of Fulfillment" embraced by community care.
