Its purpose is to ensure that people receive medical care that aligns with their values, goals, and preferences during periods of severe and chronic illness.

Furthermore, it involves communication for end-of-life care, allowing individuals to plan and decide in advance the type of care and support they desire when they lose mental capacity in the future. Through communication, family members or caregivers, healthcare professionals, and facilitators gain an understanding of the individual's decisions regarding care, medical treatment, financial matters, and end-of-life arrangements, as well as their relevant personal values, goals, and beliefs, to assist in executing the plan. This type of communication also enables individuals to rediscover what matters most to them and seize opportunities to fulfill their wishes.
